1. Responsibilities:
• Conduct regular audits of patient records to ensure compliance with organizational and regulatory standards.
• Verify the accuracy and completeness of patient information, including demographics, medical history, and treatment plans.
• Ensure that patient records adhere to established documentation guidelines, standards, and protocols.
• Identify and address any documentation errors, missing information, or inconsistencies.
• Work closely with healthcare providers, nurses, and administrative staff to address issues related to patient records.
• Identify trends or patterns in documentation errors and collaborate with relevant departments to implement corrective actions.
• Participate in quality improvement initiatives to enhance the accuracy and completeness of patient records.


• Maintain confidentiality and security of patient information following privacy regulations.
• Communicate audit findings and recommendations to appropriate stakeholders.
• Provide training sessions to healthcare staff regarding proper documentation practices and compliance requirements.
• Perform need-based audits on patient records to ensure process and quality improvement.

2. Qualifications:
Education:
• Bachelor's/Diploma in any medical course (additional education in health information management or related field is a plus).
Experience:
• Previous experience in the healthcare industry, medical records, or a related field.
• Familiarity with electronic health record (EHR) systems.
Communication Skills:
• Excellent communication skills to collaborate with various healthcare professionals.
Analytical Skills:
• Ability to analyze data and identify patterns or discrepancies.
Organizational Skills:
• Effective organizational and time-management skills to handle multiple tasks and deadlines.
